Guidelines for submission

1. Any interesting and/or complicated lung cancer cases
2. Please provide the following information:
   • Demographics: Age, Gender, Smoking History
   • Disease: Date of Diagnosis, Sites of Involvement, Stage, Histo Subtype, Molecular Profile
   • Significant Medical History
   • Lung Cancer Treatment History
   • Images
   • Question(s) to the board
